You need to venture to the Malignant Outgrowths at the end of the tunnel, where one of them will always be a Wrathful Outgrowth. Interact with the Wrathful Outgrowth to use the Wrathful Invoker. This will spawn a Wrathful Malignant enemy, which you must kill for it to drop a Wrathful Caged Heart.

Once you possess Wrathful Malignant Hearts, you can slot them into Infested Sockets on specific Season 1 gear to unlock potent bonus abilities.
